こんにちは。
最近、SALL4の抗体が使えるようになったので、病理診断においてどのような使い道があるのか調べてみました。

・Miettinen ら
・SALL4は " embryonic cell pluripotency " にかかわる転写因子であり、germ cell tumor に対する免疫マーカーとして有用だとされている。
・３２１５の腫瘍をヒト正常組織とともに調べた
・6E3 モノクローナル抗体を使用
・10th week embryo (胚) では, ovocytes, intestine, kidney, and some hepatocytes に発現
・成人の組織では、germ cell のみに発現が見られた。
・SALL4 was consistently expressed in all germ cell tumors except some
trophoblastic tumors and mature components of teratomas, where it was selectively expressed in intestinal-like and some squamous epithelia.
ほぼすべての Germ cell tumor で発現（例外は trophoblastic / teratoma の一部）
・In non germ-cell carcinomas, SALL4 was detected in 20% of cases or more of serous carcinoma of ovary, urothelial high-grade carcinoma, and gastric adenocarcinoma (especially the intestinal type).
Germ cell tumor 以外の腫瘍では、卵巣のserous carcinoma・高異型度の尿路上皮癌・胃癌（腸型の腺癌）において２０％強の症例で陽性。
・SALL4 was only rarely (≤5%) expressed in mammary, colorectal, prostatic, and squamous cell carcinomas
ごくまれに乳がん・大腸癌・前立腺癌・扁平上皮癌で陽性をしめす
・ラブドイド腫瘍 (rhabdoid tumors of kidney and extrarenal sites) や Wilms tumor では陽性。
・まれに、melanoma, desmoplastic small round cell tumor, epithelioid sarcoma, and rhabdomyosarcoma でも陽性。
・すべての血液系の腫瘍で陰性。
・結論： SALL4 は奇形腫以外の germ cell tumor で有用であり、OCT4やNANOGが陰性の 非 germ cell tumor においても診断の助けになりうる。
最後に
ということで、私は良く知らなかったマーカーですが、泌尿器病理医としては知っておくべき抗体でした。とくに精巣腫瘍の転移を疑う場合などに有用かと感じました。
