前立腺癌に対して様々な放射線治療が行われますが、その治療後の再発というのはどう判断しているのかという点をチェックしました。
NCCN (national comprehensive cancer network) のガイドラインに記載されている再発の定義をみていくと、、
RTOG-ASTRO (Radiation Therapy Oncology Group - American Society for Therapeutic Radiology and Oncology) Phoenix Consensus における定義を採用しているとあります。

いわゆるPSA再発を原文にしたがうと、"PSA persistence/recurrence" と表記されます。
① PSA increase by 2 ng/mL or more above the nadir PSA is the standard definition for PSA persistence/recurrence after EBRT with or without HT; and
② A recurrence evaluation should be considered when PSA has been confirmed to be increasing after radiation even if the increase above nadir is not yet 2 ng/mL, especially in candidates for salvage local therapy who are young and healthy.
以上が英文で記載されている定義です。
つまり、
「治療後の最低値（nadir）からPSAが２以上あがると、ホルモン治療の有無にかかわらず、放射線治療後のPSA再発 (persistence/recurrence) と判断」
されるとのこと。
前立腺摘除後の再発とは定義が異なることに注意すべきです。
